Case Management Support
We have vast experience in the provision of care and support services to case managers ranging from simple companionship and guidance through to complex care for individuals both in their own homes and out and about in the community.
Our teams can assist with long term day to day care, education and employment support or one-off ad-hoc assistance all enabling the service user to live as independently as possible. Our bespoke service ensures our members will be individually matched to your service user.
Short or long lead times can be accommodated and once instructed we can provide a complete, comprehensive managed service encompassing all aspects of the service users care and support requirements together with the provision of tailored reports.
All our members are DBS checked, qualified and provided with the highest standards of training with ongoing reviews and supervision, with the support of an experienced and dedicated on-call and senior management team. Annual leave and sickness cover to maintain that continuity of service is taken care of.
